What lands

I was a vet tech and I mislabeled two blood draws, a beagle and a shepherd that came in twenty minutes apart. I caught it while I was logging them and I told the vet before the results came back, which meant telling both owners we needed to re-draw and that we'd cover it. One of them was not gracious about it, and she was entitled not to be. After that I read the label out loud with whoever is holding the animal, every single time, and the collection order goes on the whiteboard. I still say it out loud.

Disclosed before discovery, a real person inconvenienced, a system change that stuck.

A weak answer

What most people say

I once studied the wrong chapter for an exam because I misread the syllabus. I ended up with a lower grade than I should have gotten and it was frustrating, but I learned to double-check the schedule and I put everything into one calendar after that. It hasn't happened again since.

A mistake with no victim and no disclosure. Nothing was risked in telling it.

The trap

Reading it isn't the same as saying it.
